Requirements for Admission:
- at least four years of age
- head and neck control
- meets weight guidelines (located in rider information packet below)
Please review the list of precautions and contraindications within the participant information packet linked below. If the individual has one or more of these conditions, therapeutic riding may not be recommended. If the participant meets the initial criteria, complete the application linked below.
Please review the entire participant information packet before applying. After Riding Unlimited receives the application, you will receive an email from the director with pre-filled forms to sign, date, and return. The Physician’s Referral form must be completed and signed by your physician. We should receive the Physician’s Referral form directly from the physician. All forms must be returned before you or your child can be enrolled. Registration for all riders is completed annually.
